After exhausting all possible causes of P1349 I have finally decided to replace the VVT actuator.
Apart from Onion 86 excellent write up do any members have any additional points or advice to add.
Ive come across a Toyota Service Bulletin I think via Spyderchat. It states ...After installing the VVT actuator to the camshaft " check that the camshaft timing gear assembly can move side to side and is not locked "
This seems to contradict Onion86 write up where the actuator is twisted clockwise into the locked position........and then be unable to move in either direction.

@onion86 have i misread your write up.

All and any comments welcome.

Eureka , VVT actuator replaced. Acknowledgement to @onion86. The only addition I would add to the write up, watch out when removing the 14mm bolt from the actuator, mine took a massive breaker bar to remove. Regarding getting the timing correct, do exactly as per the write up, cannot go wrong.
Let's hope this fixes the P1349 for good.
